Failure to Diagnose And Treat Pulmonary Embolus Leading to Death Of 77-Year-Old Woman

Description of Case: Decedent, a 77-year-old female, died as a result of complications from undiagnosed deep vein thrombosis and pulmonary emboli. She was admitted to a local hospital due to a recent onset of shortness of breath. Admitting diagnosis was rule out CHF and rule out pulmonary embolism. Congestive heart failure was ruled out; however, an inadequate work-up was done for pulmonary embolism and patient was discharged. One week later, patient was admitted to another hospital with further pulmonary emboli and experienced a stroke while being treated for the pulmonary embolism and thereafter died. Decedent was 77 years old and is survived by two adult children and one grandchild.
